Rules and Requirements
Nominees must be at least 6 months post op, completed treatment, chemotherapy or radiation therapy for breast cancer. Nominees must work in a healthcare field in the United States.
No purchase necessary to win. All fields in the form must be filled out, and your reason for nomination must be at least 250 characters in order to be eligible for the prize. The prize will be paid out in the form of two $500 gift cards; one for the person who nominated the warrior, and one for the nominee. Submissions must be received by 11:59 EST on October 31, 2018. Winners will be announced 11/2/18 and will be notified by email. Winner will have 7 days to claim the prize.
Elite Learning is not responsible for undeliverable email, or email that is redirected via spam or junk folders. Gift cards will be emailed to the winner and the warrior they nominated to the email address they provided no later than 7 days after acceptance of prize.
